School Application Forms
In addition to the application to the Local Authority, the school requests the completion of 2 forms which must be submitted to the school office by the application closing date published by the Local Authority.
The Supplementary Information Form (SIF) should be completed by the parent and the Parish Priest, and the Parish Priest will return the form to school no later than the application closing date.
